What is Bone Grafting?

Bone grafting is a procedure where bone is used to reconstruct areas in the jaw with inadequate bone, so that implants can be placed. The source of bone can be iether patients' own bone, of artificial bone (calcium particles).

Diagram below shows the sequence of events following a tooth loss. The alveolar bone will gradually recede, leaving bone which is much smaller than before. The reduced bone height is inadequate to support a denture, and makes implant placement impossible.

What are dental implants?

Dental implant is a titanium implant that is placed into the jaw/gum to replace missing teeth. Why titanium? Titanium is the most inert (accepted by human body) and there is no known case of titanium allergy, and it doesn't corrode compared to metal such as iron etc.

Dental implant is one of the latest innovation in dentistry. Previously, any missing teeth can only be replaced with a denture, which is although removable is troublesome and not accepted by everyone. Although dentures are still available as an easier and cheaper option of treatment, dental implant has become more popular because it is the best replacement for natural teeth.Dental implant is placed to replace the root of the tooth. Once the healing process is completed (around 3 months) the preparation can be made for construction of tooth. The teeth are normally made of porcelain, the shape and colour is 'custom-made' to complement the rest of the teeth.

There are a few factors that you have to consider when thinking about dental implant treatment. They are:

1) Treatment preferably carried out by qualified specialists - implant surgery can be complicated by inadequate bone, or other important structure such as nerves. Specialists such as Oral and Maxillofacial surgeons are able to perform additional procedures such as bone grafting without having to refer to another specialists/clinics.

2) Implants used are from internationally recognised manufacturers. The products normally have gone through clinical trials and proven to be successful, hence the cost is a bit higher. They also offers lifetime warranty, should any failure occurs the implants will be replaced free of charge. This is very important especially for those seeking treatment overseas.

3) The implants used are normal implants and not mini implants. Mini implants has a fairly limited use. Although the cost is cheaper, they are not meant to replace normal implants in most cases.

4) Surgeries are carried out in a clean, dedicated surgery room , with adequate sterilised instruments. Instruments that are not properly prepared will increase the risk of implant failures.

Temporomandibular Joint Disorder Treatment

TMJ Disorder

TMJ disorders are medical problems related to the temporomandibular joint (TMJ), the joint that connects the lower jaw to the skull. These problems can cause pain, difficulty chewing, and other issues.

You can feel your TM joints by placing your fingers directly in front of your ears and opening your mouth. What you're feeling are the rounded ends of your lower jaw as they glide along the joint socket of your temporal bone (that's the part of your skull that contains your inner ear and temple).

TMJ disorders can affect people of any age. The cause of some TMJ disorders can be traced to trauma from a severe blow to the jaw, degeneration of the joint, osteoarthritis, rheumatoid arthritis or other forms of inflammation. The causes of many cases of TMJ disorders, however, aren't always clear.Some experts believe that responses to stress or anxiety may be a primaryor contributing causal factor.

What Are the Signs and Symptoms?

Signs and symptoms of TMJ disorders may include:

  • Pain or tenderness of your jaw

  • Aching pain in and around your ear

  • Difficulty chewing or discomfort while chewing

  • Aching facial pain

  • A clicking sound or grating sensation when opening your mouth or chewing

  • Locking of the joint, making it difficult to open or close your mouth

  • Headache

  • Uncomfortable bite

  • An uneven bite, because one or more teeth are making premature contact

  • You may feel pain or tenderness, even when you aren't moving your jaw.

But in most cases, the pain or tenderness worsens when you move your jaw. Jaw clicking is common and doesn't always signal a problem. If there's no pain or limitation of movement associated with your jaw clicking, you probably don't have a TMJ disorder.

When to seek medical advice

If you have persistent pain or tenderness in your TMJ, cheek, facial pain and experience clicking or grating when you chew or move your jaw, or if you can't open or close your jaw completely, seek medical attention. Your doctor, dentist or a TMJ specialist can discuss possible causes and treatments of TMJ disorders with you.

Dental x-rays are essential, diagnostic tools that provide information not visible during a dental exam. Dentists and dental hygienists use this information to safely and accurately detect hidden problems and complete an accurate treatment plan. Without x-rays, problem areas may go undetected.

What the X-Ray can Reveal?

How often should dental x-rays be taken?

The need for dental x-rays depends on each patient’s individual dental health needs. Your dentist will recommend necessary x-rays based upon the review of your medical and dental history, a dental exam, signs and symptoms, your age, and risk of disease.

A full mouth series of dental x-rays is recommended for new patients. A full series is usually good for a few years. Bite-wing x-rays (x-rays of top and bottom teeth biting together) are taken (check-up).

For extractions, particularly wisdom teeth an OPG (orthopantomogram) is needed to examine the entire jaw. When considering orthodontics an OPG and a lateral cephalogram are also needed.

Root Canal Therapy

Root canal therapy is needed when the nerve of a tooth is affected by decay,infection or trauma. In order to save the tooth, the pulp (the living tissue inside the tooth), nerves, bacteria, and any decay are removed and the resulting space is filled with special, medicated, dental materials, which restore the tooth to its full function.

Having a root canal done on a tooth is the treatment of choice to save a tooth that otherwise would die and have to be extracted. Many patients believe that removing a tooth that has problems is the solution, but what is not realized is that extracting (pulling) a tooth will ultimately be more costly to replace.

Root canal treatment is highly successful and usually lasts a lifetime, although on occasion, a tooth will have to be retreated due to new infections.

Signs and symptoms for possible root canal therapy:

  • An abscess (or pimple) on the gums.

  • Sensitivity to hot and cold.

  • Severe toothache pain.

  • Sometimes no symptoms are present.

  • Swelling and/or tenderness.

Reasons for root canal therapy:

  • Decay has reached the tooth pulp (the living tissue inside the tooth).

  • Infection or abscess have developed inside the tooth or at the root tip.

  • Injury or trauma to the tooth.

What does root canal therapy involve?

A root canal procedure requires one or more appointments and can be performed by a dentist or endodontist (a root canal specialist).

While the tooth is numb, the tooth is isolated using a rubber dam. An access opening is made on top of the tooth and a series of root canal files are placed into the opening, one at a time, removing the pulp, nerve tissue, and bacteria. If tooth decay is present, it will also be removed.

Once the tooth is thoroughly cleaned, it will be sealed with either a permanent filling or, if additional appointments are needed, a temporary dressing/filling will be placed.

At the next appointment, usually a week later, the roots and the inside cavity of the tooth will be filled and sealed with special dental materials. A filling will be placed to cover the opening on top of the tooth. In addition, all teeth that have root canal treatment should have a crown (cap) placed. This will protect the tooth and prevent it from breaking, and restore it to its full function.

After treatment, your tooth may still be sensitive, but this will subside as the inflammation diminishes and the tooth has healed.

What are Dental Bridges?

Why do we remove wisdom teeth?

Wisdom tooth, as commonly known, is actually the third molar tooth. For most of us wisdom teeth are the last teeth in the mouth, usually erupted during the late teens or early 20's. For most of us wisdom teeth are either fully erupted, partially erupted or do not erupt at all. It is common to have some discomfort during eruption of wisdom teeth, epecially the lowers. The most common complaint is pain and swelling. This is normally followed by fever, difficulties in mouth opening, and of course eating. These will normally subside after a few days, however some will require a visit to the dentist or Oral Surgeons for cleaning and a course of antibiotics and painkillers.

Subsequently the teeth might require removal. Since most of wisdom teeth, mainly the lower, are impacted, removal means a minor oral surgical procedure.

It can be carried out under a local anaesthesia, or if you prefer general anaesthesia at a private hospital setting.

The reasons why wisdom teeth have to be removed:

  • recurrent gum infection (pericoronitis)
  • decay (caries)
  • decay of the adjacent teeth
  • cyst developing around the unerupted teeth
  • other reasons deemed removal necessary (during consultation with Oral Surgeon)

Servicenya A++ . Kes saya, cuci + scaling + cabut gigi. Total kos lebih kurang RM350. Pada pandangan saya, harga berpatutan! Lepas tu, ada juga tempat main kanak-kanak. Jadi, mungkin anak Tuan/Puan yang ingin mendapatkan rawatan pergigian, boleh juga mempertimbangkan klinik ini. Ada satu yang saya ingin cadangkan, kalau boleh, harap pihak klinik boleh mempertimbangkan untuk memanjangkan waktu operasi seperti buka sampai jam pukul 8 malam. Ini untuk memudahkan para pekerja yang bekerja sampai jam 5 atau 6 petang.

Everything's fine with me here! The clinic is very clean, the doctors and staffs have treated me very well, too. I was there about 2-3 weeks ago. I've done teeth implant for 1 tooth, and it cost me RM7,000. For all that I've surveyed so far, this is the most affordable cost for me! How nice if the clinic could send an email, or can be contacted via mobile phone, then it would be much more convenient for us.

我一直都很担心我的口腔问题,因为我的口腔里面生了一个很小的肉粒,已经一年了虽然没有消失但也没有变大。我预约咨询诊所的口腔颌面外科医生Dr.Shamsul,因为他可以帮病人做口腔手术,我有被事先告知,如果检查过后需要动大型的手术可能要到Dr.Shamsul任职的医院。经过了检查之后,Dr.Shamsul告诉我,我的口腔肉粒是良性的,是因为口腔摩擦导致的,割除它是一个小手术,可以直接在诊所进行。整个手术过程,麻醉、切除、缝针,只用不到15分钟的时间,医生跟细心地讲解我的情况,提醒我伤口的照顾事项,也安排我下个星期复诊,检查我的复原情况。诊所的位置在店屋楼上,有点难找,不过诊所的内部环境很整洁舒适。必须要提的是,诊所的护士的态度很礼貌友善,还有必须给牙科专科Dr.Shamsul一百个赞!因为他非常亲切和细心,让我放下了本来看牙医的紧张心情。(唯一可惜的地方是,医生和护士都是讲英语和马来语的,我这个“华语人”虽然听得懂医生说什么,但是不太敢问医生问题。)

Service was good and nice. Had received scaling treatment. They do it nicely and gently. Highly recommend this clinic to others. Hope they can maintain their service when i received the treatment after this, maybe about six month later? Besides, I hope clinic can lower the treatment cost.

Saya betul-betul puas hati terhadap segala perkhidmatan dan rawatan di klinik ini. Sangat baik. Jika dibandingkan dengan klinik biasa lain, saya suka klinik ini, terutamanya Dr. Norrazian yang mengendali kes saya. Kawan-kawan lain mengambil berat terhadap keadaan saya, mereka tertanya-tanya sama ada sakit atau tidak semasa rawatan dijalankan. Tidak dapat dinafikan bahawa saya berasa selesa sekali dan tidak berasa sakit langsung sepanjang proses rawatan. Ini membuatkan saya mengesyorkan klinik ini kepada kawan-kawan yang ingin menerima rawatan pergigian.

"My dentist in the USA said that I had to have major work after all the years of neglect. Below are what I was told I needed to do:a. 2-3 root canals. (quoted $2-3K/each)b. Orthodontic work to straighten premolar tooth that was collapsing due to an old missing tooth. ($8K-10K).c. Extract my wisdom tooth ($500-1000)d. Dental implants ($3K to 4K/each depending on the services included in procedure)e. Bone grafting ($500-1000/each)Total cost to fix my dental problem could run as high as $30K.Dr. Shamsul and Dr. Azian gave me a comprehensive rundown and my options:a. No root canal necessary after they drilled into my teeth at the first meeting @11am. Just fillings,xrays and descalling/cleaning ... as well as mopping up work on some deteriorating fillings. Took 1 1/2 hour, relatively painless for around MR$600 or about USD$150.b.Dr. Shamsul wasn't available until 4pm for my next appontment. So met with some friends for lunch, and came back around 3.30pm to miss the traffic. He told me that both molar and wisdom tooth were loose, and it was a lost cause to do all the orthodontic work. His recommendation was to extract both teeth, and do two dental implants for about MR$8.8K/each or around USD$2.2K/each. (With the bone graft)c. Since I was prepared to pay up to USD$10K for everything, this was a nice surprise. But timing was an issue since I was only going to be in Kuala Lumpur for 9 days. He said he could do the work (post installation) immediately and it would take between 90-120 minutes. But that I would have to come back in about 6 months to install the crown.d. Procedure was done with minimal pain, gave me all the medication without having to drop by a pharmacy, left the office and walked to the LRT station to get back to my hotel. (short 10 minute walk to the LRTR) Total came up to MR$17,500 or about USD$4400.e. Have my last appointment to check on the healing status 5 days after the procedure. But will be coming back for the crown installs in January 2017 or whenever I can get a cheap flight.12 hours after the procedure, I still have some tenderness, slight pain, swollen gums but everything was manageable. Didn't stop me from working at midnight until 6am.Overall visit cost me MR$18,100 which I estimate about USD$4550. My flight was about USD$650 and 4-star hotel with breakfast cost about $65/day. But I also have a lot of business meetings after the dental work. Time will tell if all goes as plan but so far I am very happy with the whole experience and the competence of the Bangsar Utama Dental Specialist. I chose Bangsar Utama Dental Specialist Center after reviewing the feedback on your website, the location of the office since it was within 15 minutes of my hotel via taxi (Concorde KL). And they were the first one to respond as well as the most responsive to my questions. I also checked the bio of the doctors there as well as the services they provided. It looked like a one-stop-dental-shop ... and they also made the crowns onsite. My assumption was that they would have done quite a few implants/crowns and had a lot of experience doing complicated dental work.I have been to some US dental schools to save a buck. Doesn't give me too much confidence but all of them turned out well except for the long wait and long consultation session as well as some mistakes that the professors corrected.The team at Bangsar Utama Dental Specialist were cordial, professional and very competent. I had a hard time finding the clinic in the non-descript building, and even the taxi driver and the locals couldn't tell me where it was located. But since I was an hour early, I did manage to find it with about 30mins to spare. It helps that I can speak the local language. Otherwise, get the taxi drive to drop you off outside the building where the is a small sign over the door. Cab/taxi fee came up to about USD$6.I loved that the place had great AC, you have to remove your shoes (somehow it felt more relaxed), non rushed environment and extremely friendly people. They even have a great coffee shop downstairs if they get busy. (loved the nasi lemak).Area look safe but then again I do blend in with the locals. Otherwise, pre plan for the taxi to pick you up outside the building or get the uber or my teksi app for your phone."

CAD system was amazing to watch
"I had about 15 fillings that needed to be sorted out and 4 root canals and 3 crowns. The cost in Australia would have been about $14,000 but if I had of shopped around perhaps as low as 12,000, went to bangsar utama and they sorted everything out the total cost of $3300, it has been a couple of weeks now and no problems everything fits and feels good. No problem, the dental clinic was a little different to what you expect in Australia, most everything is clean it's just not "as clean" but that does not mean it's not acceptable and okay, some of the fixtures and fittings the furniture etcetera is a bit old but it's all kept in good repair and just not brand-new, the equipment itself seems all up-to-date, in particular this clinic have CAD system for scanning and making crowns and that was pretty amazing to watch and the fit was absolutely perfect without any modification."

Highly recommend
The surgeon maintained very good and timely communications with me throughout both the booking and treatment processes, meaning we both knew exactly what to expect. Though I presented with complications during the surgery, both the surgeon and senior dentist walked me through the treatment options, allowing me to make informed decisions. All staff at the clinic were friendly and professional, and made me feel at ease throughout the protracted surgery and post-surgical care. I highly recommend this dental centre.

Very friendly and helpful
I am midway through some major restorative work that includes extractions, fillings, a bone graft and implants. I received a comprehensive treatment plan and detailed quotation following X-rays and a thorough check up. The team is very friendly and helpful. I have had the extractions and fillings completed including some excellent work on a broken front tooth which has greatly improved my appearance. I am very happy with the work to date and as a reluctant dental patient I am looking forward to returning for my implants. The surgery can be a little difficult to locate and I recommend using a GPS enabled smartphone as many taxi drivers do not use a GPS and struggle in the back streets of Bangsa Utama.

Porcelain Veneers
They gave me a new Hollywood smile with sparkling white veneers on my upper front. I paid 1/3 of what I would have to pay in Sydney and the service was excellent. I am going back in July to get all the rest of my teeth veneered/ crowned with the same lovely sparling white porcelain. They took a model of my teeth first (Wax up) and they contoured the model to show me exactly what the new veneers would look like. They took 3mm off the front and gave me temp plastic coverage then 7 days later they fitted my veneers. It looks awesome!

Dental Crowns
I have been looking to change the appearance of my teeth with either veneers or crown, and I have been to 4 different dentist to seek opinion, Bangsar Utama is the last of the 4 clinics that i went, which I have decided to have the procedure with. Dr Azian was the only one that has advised me to maintain as many of my original good teeth to maintain yet achieving the aesthetic effect. She also took time to do an impression of what she planned to do with my teeth so that I can make a better decision which I was very, very impressed with, as none of the other doctor took the trouble to offer this. The procedure was a pain like all the usual dental work, No one can escape!! Dr Azian is very patient & gentle, and quite a perfectionist. She sent back my crown during the fitting as she was not pleased with the ceramic work of the crown, we waited for another week for the vendor to re-do the crown. All in all, I am very pleased with Dr Azian, and would recommend anyone to her for dental work.
