Sometimes people are too young to consider surgery or simply do not want surgery. Facial volume may be needed but excessive volume is not desirable and lift is preferable. Whilst permanent threads have been in existence for two decades (e.g. Aptos and Silhouette Lift), with Silhouette Soft the thread is fully absorbable and requires no incision or stitches.

What is Silhouette Soft?

Silhouette Soft® is produced in the United States by the same company which introduced the Silhouette Lift® sutures in the plastic surgery market in 2006.  Silhouette Lift® threads are suspension sutures which are used in cosmetic and reconstructive surgery.

They have received approval from the US FDA (since 2006) and they are CE marked for the European market.  Since 2007 more than 20.000 Silhouette Lift® procedures were carried out successfully and presently every 15 minutes somebody somewhere in the world is being helped with Silhouette Lift®.

In October 2012 Silhouette Soft® was approved for use in the European market as a “spin-off” of the Silhouette Lift® sutures: they are shorter, with fewer cones and without the need to be stitched to the skin on either end.  They are best described as polylactic acid bi-directional bio-resorbable sutures with cones.

Unlike other existing threads, Silhouette Soft® is made out of a natural component: polylactic acid (PLA), which is derived from renewable resources such as corn starch or sugarcane. Being able to degrade into harmless lactic acid, PLA is used for medical implants in the form of screws, pins and rods (and now also lifting threads). PLA breaks down gradually over a period of 6 to 24 months. This gradual breakdown is desirable for creating a support structure.

How does it work?

Combating the effects of aging means restoring smoother and more toned skin, but also a more shapely face.  To meet this dual requirement Silhouette Soft® combines two effects: lifting and regenerating:

The lifting effect is immediate and discreet as it is the result of compressing and elevating tissue at the time of adjusting the suture.  After inserting the suture, the doctor applies slight pressure on the treated area to reshape the surface of the skin and make it visibly smoother.

The regenerative effect is gradual and natural: polylactic acid (PLA), the principal component of Silhouette Soft® is particularly biocompatible with the human body and completely biodegradable. Once the suture is applied, the PLA acts on the deeper layers of the skin and naturally stimulates the production of collagen. This action which continues over time helps increase the volume of saggy areas and restores shapeliness to the face gradually and perfectly naturally.

Who should go for this treatment?

Silhouette Soft® is recommended for men and women of 30 and over who are trying to combat effectively the signs of aging, which do not want any heavy and invasive treatment, but are looking for real and natural results.

Silhouette Soft® treatment is suitable for all skin types – testing for allergic reactions proved negative in clinical studies.  Nevertheless the use of Silhouette Soft® is not recommended if you are allergic or sensitive to biomaterials.

What is the procedure of this treatment?

The insertion of the suture in the subcutaneous (adipose) skin layer occurs through an entry point in the middle of the suture pattern. The entry point enables the insertion of each half of the suture respecting the appropriate orientation of the cones. After the insertion of the sutures in the treated area a manual compression of the adipose tissue is performed to obtain the full and immediate result.

A light local anaesthesia is required only at the entry and the exit points of the sutures.  The sutures are inserted under sterile conditions through the skin in the subcutaneous tissue. The procedure is performed at the office and does not require any surgical incisions or stitches.

How long do the results last?

The results may last average between 12 to 18 months, varied with each individual. A healthy lifestyle and good skincare regime can prolong the effect of the treatment.

What to do before the treatment?

No special preparation is required prior to Silhouette Soft® treatment.

However, we recommend that you discuss your expectations and ask your doctor any questions you have before starting treatment. Inform the doctor of any treatment you have already received and of any medicines you are currently taking.

Is there any downtime after the treatment?

After Silhouette Soft© treatment, as with any cosmetic treatment, some slight swelling, redness or bruising may sometimes be observed, but these disappear within days.

There is sometimes a slight wrinkling of the skin (required for optimisation of the treatment) which disappears very quickly.

Recommendations for good post-treatment care:

  • During the week after treatment, do not undergo any dental surgery, and avoid facial and beauty treatments.

  • Avoid all contact sports for the first few weeks.

  • Preferably sleep on your back.

  • In the event of pain, take a pain reliever according to your doctor’s prescription.

Can I undergo for other cosmetic procedures after the treatment?

Yes, you can. Silhouette Soft constitutes a perfect addition to dermal fillers (Juvederm) and botulinum toxin (Botox). The three different procedures together act as a complete and very efficient armory in the war against aging as they complement the effect of each others and work synergistically.

Silhouette Thread Lift + More

Maybe you just want to look on the outside the way you feel on the inside.

The Silhouette Sutures has clear, absorbable hollow cones which allows for more tissue growth and long-lasting anchoring. The Silhouette Sutures are not cut (or barbed) sutures. Silhouette Sutures are much stronger and since the cones are hollow and round they will absorb completely over time and be less visible. There will be no migration.

The Silhouette Lift is a mid face lift that uses the new Silhouette Sutures to lift sagging tissue in the mid-face area. It can also be used to lift the jowls. They are an entirely new, FDA approved material used for lifting soft tissues in the facial areas. They are made of polypropylene strands, which allows for better results than with the “barbed” sutures. There is less down time involved with this lift.
Silhouette sutures do not have sharp barbs, which can weaken the suture. A barbed suture is created when cuts are made in the shaft of conventional sutures, which then weakens the inherent strength of the thread itself. Silhouette sutures are made from 3-0 polypropylene substrate, which allows for smaller knot tying in the temporal area, and also utilizes clear, flexible, absorbable cones. Over time the cones are completely absorbed and tissue grows around and through the small knots to allow for long-lasting tissue suspension.

Typically anyone between ages 40-60 years old will benefit from this lift. There must be an obvious drooping of the mid-face soft tissues in order to be a good candidate and see optimum results. Men and women alike have participated in the rigorous 3 year trial period – using the Silhouette Sutures lift – both achieved outstanding results.

The advantages include (but are not limited to) the following:

• There are no barbs to cause weakening of the suture.
• The hollow cones of the Silhouette sutures allow for much stronger anchoring.
• It is much less invasive than a full facelift so there is less trauma and downtime.

In most cases, there is minimal discomfort, bruising and swelling. Ice compresses are recommended for the first 24-48 hours. After a Silhouette thread lift, patients can usually return to work or normal activities in a few days to one week.

Duration varies depending on the age of the patient at the time of the procedure, the degree of sagging, and the number of threads used. Additional threads may be added as needed.

The Silhouette Thread Lift procedure is a simple procedure to perform. Side effects can include some minor swelling and bruising. Complications are very rare, but can include a small pucker at the entrance of the thread, or the thread can protrude slightly from the skin. If such side effects occur, they are easily treatable by trimming the excess thread or massaging the skin. Since this technique is entirely reversible, unwanted effects are usually easily corrected under local anesthesia.

Good candidates for Silhouette Thread Lift are those patients who are starting to see some droopiness of the cheeks, jowls and/or neck and would like to see a lift effect, but do not want the risk, scars, or recovery of a real facelift, or necklift operation. It is a procedure that is typically suited towards younger and middle-aged patients

Poor candidates include those patients with unrealistic expectations, have uncontrolled medical illnesses that affect wound healing, are grossly overweight (with heavy faces) or are excessively thin, loose skin. The ThreadLift may be combined with other procedures, such as

Botox, Injectable Fillers, Laser Skin Regeneration.
Results can last for several years, again depending on various factors such as area treated and the natural aging process.

After you are prepped and made completely comfortable with local anesthesia and a relaxant if needed, the doctors will make a small incision in the hairline area through which he or she will draw the needle and Silhouette Suture to the mid-face area. The sutures are deployed in the soft tissue of the malar and peri-oral areas, placed slightly parallel to each other. The Silhouette Sutures are then pulled backwards until the cones engage the tissues of those facial areas. This engagement suspends and lifts them to a higher location. The sutures are then fixed in pairs in the deep tissue area. The lift is instantaneous and longer lasting than with all other previous techniques and procedures. The sagging mid-face tissue is elevated to a new, higher position – ensuring you a more youthful, long lasting appearance. This can also be used to lift the jowls.
